ABSTRACT

Purpose@#This study aimed to identify the prevalence of sarcopenia, obesity, and sarcopenic obesity and their association to radiographic knee osteoarthritis (OA) and knee pain in Korean menopausal women. @*Methods@#This study cross-sectional study utilized the data obtained from Korean National Health and Nutrition Examination Surveys (KNHANES) 2009-2011 which included 4,362 menopausal women. The participants were categorized based on their body composition and the prevalence of radiographic knee OA and knee pain were calculated. The effect of hormone therapy (HT) was analyzed as subcohort analysis. @*Results@#The prevalence of radiographic knee OA, knee pain, and both radiographic knee OA and knee pain were all highest in sarcopenic obese group and lowest in non-sarcopenic non-obese group. Among the non-sarcopenic people, the obese people showed higher ratio of only radiographic knee OA (57.64% vs. 41.54%, p < 0.001). When sarcopenic, the coexistence of obesity presented significantly higher ratio of radiographic knee OA, knee pain, and both radiographic knee OA and knee pain compared to sarcopenic without obesity (61.49% vs. 41.82%, 39.11% vs. 27.61%, 32.04 vs. 17.60%, all p < 0.001).Regarding the use of HT for longer than 1 year was not associated with statistically different ratio of neither radiographic knee OA nor knee pain. @*Conclusion@#The obesity in terms of sarcopenia have greater effect on knee OA compared to the obesity without sarcopenia and HT use of longer than 1 year is not associated with the prevalence of knee OA. Therefore, more preventive effort to knee OA should be focused on reducing body fat and increasing muscle in the postmenopausal women.

ABSTRACT

Purpose@#This study aimed to investigate whether the number of parity is associated with the prevalence of sarcopenia in postmenopausal women. @*Methods@#This study was performed using data from the 2010-2011 Korean National Health and Nutrition Examination Survey that included 1,338 postmenopausal women aged 46-70 years. The prevalence of sarcopenia was analyzed according to the number of births. Modifiable risk factors were evaluated to determine the association of parity with sarcopenia in susceptible population. @*Results@#The sarcopenia group (n=343) had lower number of parity, lower body mass index, more frequent previous history of diabetes mellitus, higher Homeostatic Model Assessment of Insulin Resistance (HOMA-IR), and higher education level than the non-sarcopenia group (n=995). The sarcopenia risk was significantly lower in postmenopausal women with a higher number of parity (≥3 births) (model 1: odds ratio [OR]=0.308; 95% confidence interval [CI]=0.115–0.827, p=0.0194; model 2: OR=0.269; 95% CI=0.109–0.66, p=0.0042; model 3: OR=0.640; 95% CI=0.428–0.957, p=0.0295; model 4: OR=0.636; 95% CI=0.413–0.980, p=0.0403). In subgroup analysis of the lower parity group, moderate aerobic activity was associated with a lower sarcopenia prevalence (OR: 0.52; 95% CI: 0.317–0.852; p=0.0095). @*Conclusion@#A lower number of parity increases the risk of sarcopenia in postmenopausal Korean women. Moderate aerobic activity may be effective in preventing sarcopenia in postmenopausal women with lower parity who are more susceptible to sarcopenia.

MicroRNA (miRNA) expression was analyzed by quantitative real-time polymerase chain reaction. To assess the effects of miR-139-5p on cultured leiomyoma cells, cell migration, collagen gel contraction, wound healing, and the expression levels of hallmark proteins were evaluated in cells transfected with a miR-139-5p mimic. @*Results@#The expression of miR-139-5p was significantly lower in leiomyoma tissues than in matched smooth muscle tissues. Restored miR-139-5p expression in miR-139-5p mimic-transfected human leiomyoma cells resulted in decreased contractility of the ECM and cell migration. In addition, upregulation of miR-139-5p decreased the protein expression of collagen type 1 and phosphorylated p38 MAPK. @*Conclusion@#Expression of miR-139-5p is downregulated in leiomyoma cells and modulation of miR-139-5p may be involved in the pathogenesis of leiomyomas through the regulation of collagen type 1 and phosphorylated p38 MAPK. Therefore, miR-139-5p is a potential therapeutic target for leiomyoma.

Objective@#To evaluate the accuracy and quality of Korean videos associated with restless legs syndrome (RLS) on YouTube. @*Methods@#A YouTube search was performed on April 1, 2020 using the term “restless legs syndrome” in the Korean language. Two reviewers coded the source, content, and demographics of the included videos. Video quality was assessed using the modified DISCERN (mDISCERN) instrument. @*Results@#Among the 80 videos analyzed, 44 (55.0%) were reliable, and 36 (45.0%) were misleading. There was a trend toward a higher number of mean daily views in the misleading videos than in the reliable videos. Most of the misleading videos (72.2%) advocated complementary and alternative medicine as a primary treatment for RLS. Although the reliable videos had higher mDISCERN scores than the misleading videos, the overall quality of the reliable videos was low. @*Conclusion@#Many Korean videos regarding RLS on YouTube involve a risk of exposure to misinformation and are of unsatisfactory quality.

ABSTRACT

Pain in Guillain-Barré syndrome (GBS) is known as a common symptom, experienced by about 72% of patients. Various types of pain are associated with GBS, including paresthesia, dysesthesia, radicular pain, meningism, myalgia and visceral pain. Pain in GBS can vary from mild to severe, often under-recognized and poorly managed. This article reviews the various pains associated with Guillain-Barré syndrome and their management.

ABSTRACT

Background@#Recent studies have shown that oral administration of probiotics may improve the immune imbalance caused by dysbiosis of the gut microbiome in atopic dermatitis (AD). This study aimed to investigate the clinical and immunological effects of Lactobacillus pentosus in children with mild to moderate AD. @*Methods@#Children aged 2–13 years with AD were randomized to receive either 1.0 × 1010 colony-forming units of L. pentosus or placebo, daily, for 12 weeks. The clinical severity of AD and transepidermal water loss were evaluated. Blood eosinophil counts, serum total immunoglobulin E (IgE), and cytokine levels were measured. The diversity and composition of the gut microbiota were also analyzed. @*Results@#Eighty-two children were recruited, and 41 were assigned to the probiotics intervention group. The mean scoring of atopic dermatitis (SCORAD) indices at baseline were 30.4 and 34.3 for the probiotics and placebo groups, respectively. At week 12, the mean indices were 23.6 and 23.1 for the probiotics and placebo groups, respectively. Clinical severity decreased significantly over time in both groups, with no significant difference between the two groups. In both groups, there were no significant differences in cytokine levels, microbial diversity, or the relative abundance of the gut microbiota at week 12 compared with the corresponding baseline values. The mean subjective scores of SCORAD indices after intervention for the probiotics group were significantly lower than those for the placebo group in IgE sensitized AD (P = 0.019). @*Conclusion@#Our results show improved symptoms in the probiotics and placebo groups, and we could not find additional effects of L. pentosus in AD. However, the mean subjective scores of SCORAD indices for the probiotics group are significantly improved compared with those for the placebo group in allergen-sensitized AD.

ABSTRACT

PURPOSE: Recently, the prevalence of allergic rhinitis (AR) in Korean children has been increased. The aim of this study was to investigate the clinical characteristics of rhinitis and to compare clinical parameters between AR and nonallergic rhinitis (NAR) in children.METHODS: We retrospectively reviewed the medical records of 1,034 children under 18 years of age who visited Korea University Anam Hospital for rhinitis symptoms from January 2008 to December 2017. Clinical data, including clinical features, comorbidities, blood test results, allergen sensitization profile, and pulmonary function test parameters, were collected.RESULTS: Among the 1,034 children with rhinitis, 737 (71.3%) were AR and 297 (28.7%) were NAR. The prevalence of AR gradually increased with age. The median levels of eosinophil count (4.1%), serum total IgE (204.4 IU/L), eosinophil cationic protein (ECP) concentration (17.9 µg/L), and fractional exhaled nitric oxide (FeNO) (22.0 ppb) were significantly higher in children with AR than in those with NAR. The sensitization rate to the inhalant allergens increased with age; however, food allergen sensitization rate tended to decrease. Median levels of eosinophil count, total IgE, ECP, and FeNO were significantly higher in the poly-sensitized group than in the mono-sensitized and nonsensitized groups.CONCLUSION: More than 70% of Korean children who have rhinitis symptoms are AR. Children with AR more likely to have higher levels of FeNO and bronchial asthma. Poly-sensitized children showed increased rates of atopic dermatitis and bronchial asthma.

ABSTRACT

Purpose@#As dementia progresses, cognitive function decreasing leads to memory loss, speech degradation, time and space degradation and judgment degradation, which causes difficulties in carrying out tasks related to daily life. It was said that community-based non-drug intervention therapy for early dementia patients was important to participate in entertainment treatment, including activities such as awareness and exercise therapy, exercise rehabilitation, aerobic exercise, and art. @*Methods@#This study conducted 15 experimental and 15 control groups(experimental group : Fumanet exercise, control group : general occupational therapy) for eight weeks at the Daycare Center in Gyeonggi-do to find out the impact of the Fumanet exercise program on cognitive and depression functions of the elderly. The pre-post evaluation used KGDS, MMSE. @*Results@#There were significant differences between the two groups in the function of menopause, memory recall, attention concentration and calculation, and depression, and no significant results were obtatined in memory registration, language function, understanding and fracture. The Fumanet movement was judged to be effective in improving cognitive function and reducing depression for the elderly with dementia.Conclisions: The Fumanet movement was judged to be effective in improving cognitive function and reducing depression for the elderly with dementia.

ABSTRACT

PURPOSE: With the increase of going out to eat, reducing the sodium in restaurant foods has a crucial impact on reducing sodium intake. This study aimed to assess the current status and perceptions of restaurant staffs related to reducing sodium use in restaurants. METHODS: Restaurant managers and chefs (n = 312) in Seongnam area completed a questionnaire on the current status related to sodium use, the barriers to practice for reducing sodium use, support needs, and the practicability of methods for reducing sodium use in restaurants. RESULTS: The percentage of restaurants in the preaction stage (including the precontemplation, contemplation, and preparation stages) for reducing sodium use was 79.7%. Logistic regression analysis showed that measuring salinity while cooking was associated with measuring seasoning (OR, 4.761; 95% CI, 2.325 ~ 9.751), action/maintenance stages of behavior change (OR, 2.829; 95% CI, 1.449 ~ 5.525) and providing salinity information of restaurant foods (OR, 6.314; 95% CI, 2.964 ~ 13.45). Maintaining taste and hindering the cooking process were the main barriers to reduce sodium use. The total practicability of actions for reducing sodium was higher in staffs who worked in restaurants that measured seasoning and salinity while cooking (p < 0.05 and p < 0.01, respectively). The hardest item to practice was ‘purchase foods after comparing sodium content in the nutrition labeling’. ‘Avoid serving salt-fermented foods as side dishes’, ‘serve small portions of kimchi and less salty kimchi’, and ‘put up promotional materials for reducing sodium intake’ were selected as easy items to perform. The majority (82%) was willing to reduce sodium in restaurant foods under the support of local government and they desired the promotion of participating restaurants and education on cooking skills to reduce sodium. CONCLUSION: Measuring seasoning and salinity while cooking is a meaningful practice that is associated with stages of behavior change and the practicability of actions for reducing sodium. It is necessary to provide support and education with a gradual approach to staffs for reducing sodium in restaurant foods.

ABSTRACT

OBJECTIVES: The purpose of this study was to investigate the effect of psychopathology on the quality of life in dermatological outpatients.METHODS: A sample of 151 female dermatological outpatients was compared with a control group of 200 females. Symptom Check List-90-R(SCL-90-R) was used to screen for psychopathology and Skindex-29 for quality of life. We compared the scores of SCL-90-R and Skindex-29 between the two groups and examined the correlation between SCL-90-R and Skindex-29 in the patient group. We analysed the subscales of SCL-90-R that affect Skindex-29.RESULTS: There was statistically significant higher in the score of SCL-90-R in patient group than control group. In high score group of Skindex-29, the score of SCL-90-R was significantly higher than in low score group of Skindex-29. The correlation between SCL-90-R and Skindex-29 was positively correlated with moderate relationship on all subscales except paranoid ideation. Somatization and psychoticism of SCL-90-R were the factors affecting on the total score of Skindex-29.CONCLUSION: The results of this study suggest that psychological distress of the dermatological outpatients is more severe than control group, and somatization and psychoticism are suggested as predictors of the quality of life of dermatologic patients.

OBJECTIVES: This study was conducted to examine the relationship between white rice and Kimchi intakes and the risk of metabolic syndrome (Mets) in Korean adults. METHODS: Dietary intake and health data of 8289 subjects aged 19 years and over from the 2013–2015 Korea National Health and Nutrition Examination Survey (KNHANES) were used. Daily total intake of white rice and Kimchi was assessed by 24-hour recall data. Multivariate logistic regression analysis was used to estimate the risk of Mets according to the daily intake of white rice and Kimchi. RESULTS: The highest intake of white rice and Kimchi was associated with a higher risk of metabolic syndrome (Q1 vs Q5, multivariable adjusted OR=1.45, 95% CI: 1.03–2.03) in women. In addition, a significantly increased risk of elevated blood pressure (multivariable adjusted P for trend 0.0459) was associated with a higher intake of white rice and Kimchi in women. There was no significant trend in the risk of metabolic syndrome according to the intake of white rice and Kimchi in men. CONCLUSIONS: A higher intake of white rice and Kimchi was only associated with an increased risk of metabolic syndrome in women indicating it is necessary to consume more various food groups beside white rice and Kimchi, especially in women.

ABSTRACT

Atopic dermatitis (AD) is a chronic inflammatory skin disease in children. Patients with AD experience a high rate of colonization of the skin surface by Staphylococcus aureus. Because of a skin barrier defect, there is a potential risk of staphylococcal invasive infection in patients with AD. Here, we present 2 cases of breast abscess caused by S. aureus in 2 adolescent girls with severe AD. Methicillin-sensitive S. aureus was identified from the breast abscess material. They were treated with appropriate antibiotics, however surgical drainage of the abscess was needed in case 1. Identical strains were found from the breast abscess material as well as the lesional and the nonlesional skin of the patients through matrixassisted laser desorption/ionization time-of-flight analysis. We characterized the differential abundance of Firmicutes phylum in patients' skin in microbiota analysis. In particular, S. aureus, a member of Firmicutes, differed significantly between the lesional and the normal-appearing skin. Our cases demonstrate the potential severity of bacterial deep tissue infection in AD and the dysbiosis of skin microbiota may be involved in inflammation in AD.

ABSTRACT

OBJECTIVE: This study aimed to identify the core symptoms of delirium, particularly in elderly people associated with major risk factors, using the Korean version of the Delirium Rating Scale-Revised-98. METHODS: The study sample consisted of 200 patients (mean age: 72.7±3.9 years, male: 68.5%) who had been diagnosed with delirium. Exploratory factor analysis was used to investigate the factor structure, and confirmatory factor analysis was used to evaluate the goodness of fit of the results. RESULTS: The results demonstrated three core domains of delirium in the elderly patients: 1) the cognitive domain (e.g., language, thought process, orientation, attention, long-term memory, and visuospatial ability); 2) the circadian domain (e.g., sleep-wake cycle and motor behavior); and 3) the short-term memory domain (short-term memory). These results were confirmed by confirmatory factor analysis. CONCLUSION: The findings of this study suggest a theoretical domain structure for delirium in elderly patients.

ABSTRACT

OBJECTIVE: This study compared the symptomatic emotional traits of elderly South Korean patients with hwa-byung and those with depression. METHODS: We enrolled 58 patients with hwa-byung, 180 patients with depression, and 181 healthy control subjects. All participants completed the Hwa-byung Scale, Hamilton Depression Rating Scale (HDRS), Beck Depression Inventory (BDI), Geriatric Depression Scale (GDS), State Trait Anger Expression Inventory (STAXI), Reaction Inventory (RI), and Aggression Questionnaire (AQ). Chi-square tests and a one-way analysis of variance with Games-Howell post-hoc tests were used to compare demographic variables and scores. RESULTS: A binary logistic regression analysis was used to examine risk factors for hwa-byung. Scores in the hwa-byung group were higher than those in the depression group for the HDRS; BDI; GDS; trait anger STAXI subscale (trait anger temperament and trait anger reaction); state anger and anger expression STAXI subscales (anger-in, anger-out, and anger control); physical and verbal aggression as well as anger and hostility AQ subscales; and disturbance, embarrassing circumstances, personal disrespect, and unpleasant encounters RI subscales. A binary logistic regression analysis demonstrated that the state anger STAXI subscale, verbal aggression and anger AQ subscales, and unpleasant encounters RI subscale were significantly associated with hwa-byung. CONCLUSION: Elderly patients with hwa-byung had more severe anger traits and states as well as higher depression severity compared to those diagnosed with clinical depression. Excessive anger and anger reactivity to unpleasant factors may be risk factors for hwa-byung, whereas the appropriate expression (rather than suppression) of anger may decrease the risk of hwa-byung.

ABSTRACT

PURPOSE: Recent data indicate that sensitization to mold contributes to the severity and persistence of asthma. In this study, we investigated the relationships between sensitization to mold and lung function parameters in children with asthma. METHODS: We retrospectively reviewed clinical data from 551 asthmatic subjects. We selected subjects who met clinical diagnostic criteria of asthma. Their spirometry, methacholine challenge tests, and measurements of blood eosinophils, serum IgE, eosinophil cationic protein (ECP) and fractional exhaled nitric oxide (FeNO) results were included. Skin prick testing (SPT) results with 13 common aeroallergens in Korea including house dust mites, animal dander, pollen, cockroach and mold were reviewed. Subjects were divided into 3 groups according to their SPT results. Subjects who showed no positive result to any aeroallergen were designated as group 1 (non-sensitized). Group 2 represented subjects who were sensitized to aeroallergens other than mold (other allergen-sensitized) and group 3 included subjects who were sensitized to mold allergens (mold-sensitized). RESULTS: Among the 551 asthmatic subjects, 67 (12.2%) were sensitized to mold and 366 (66.4%) were sensitized to other aeroallergens. The log mean IgE levels were higher in groups 2 (5.96±1.14 IU/mL) and 3 (5.81±0.97 IU/mL) compared to group 1 (3.88±1.68 IU/mL). Blood eosinophils, ECP and FeNO concentrations were significantly higher in groups 2 and 3, but no significant difference was found between the 2 groups. The mean FEV1 value was significantly lower in group 3 (86.9±12.1%pred) than in groups 2 (92.0±14.8%pred) and 1 (93.4±15.4%pred). The log mean methacholine PC20 was significantly lower in group 3 (0.08±1.91 mg/mL) than in groups 2 (1.31±1.69 mg/mL) and 1 (2.29±1.66 mg/mL). CONCLUSIONS: We observed a differential association between mold and other aeroallergen sensitization, and severity of asthma. Sensitization to mold is associated with lower lung function and increased airway hyper-responsiveness in children with asthma. Mold sensitization could be an important factor determining asthma severity particularly airflow limitation in children.

ABSTRACT

OBJECTIVE: This study aimed to evaluate the relationship between suicidal ideation and its associated clinical factors among community-dwelling elderly in Korea. METHODS: A cross-sectional survey was conducted using the data obtained from the face to face interviews of 1,500 (mean age : 74.41±6.44, male : 41.0%) elderly people aged 65 or above living in the community. The questionnaire used in this study was composed of detailed questions about the clinical characteristics including sociodemographic, life style, physical status, and psychiatric factors. In order to assess the suicidal ideation, the subjects were asked questions about the suicidal idea in the past year. RESULTS: A total of 156 (10.4%) elderly reported the suicidal ideation over the last year. Multiple logistic regression analysis revealed significant correlation with dissatisfaction with life [odds ratio (OR) 4.41, 95% confidence interval (CI) 1.55-12.60], depression (OR 2.87, 95% CI 1.62-5.07), cognitive decline (OR 2.38, 95% CI 1.38-4.10), poor perception of health (OR 2.34, 95% CI 1.04-5.24), and insomnia (OR 2.24, 95% CI 1.31-3.84). CONCLUSION: These results showed the several clinical factors associated with suicidal ideation among community-dwelling elderly living in Korea. The present study suggested the necessity for the various aspects of approach.

ABSTRACT

BACKGROUND: Moyamoya disease is characterized by a progressive stenosis or occlusion of the intracranial internal carotid artery and/or the proximal portion of the anterior cerebral artery and middle cerebral artery. Whether the onset time was childhood or adulthood, the bony carotid canal diameter might be different, but reflects the size of internal carotid artery passing through the bony carotid canal. In this study, we aimed to identify the relationship between bony carotid canal diameter and clinical manifestation. METHODS: 146 consecutive patients diagnosed with moyamoya disease by brain imaging studies were included. We measured the diameter of a transverse portion of bony carotid canal on bone window of a brain computed tomography(CT) image. Patients were divided into two groups, ischemic or hemorrhagic stroke according to clinical manifestation. As a result, 115 patients were included. The Suzuki stage was used as criteria for disease progression. RESULTS: Bony carotid canal diameter was 3.6 ± 0.5 (right) and 3.6 ± 0.4 (left) in the hemorrhagic stroke group, and 3.7 ± 0.4 (right) and 3.6 ± 0.4 (left) in the ischemic stroke group. The bony carotid canal diameter of the moyamoya vessels (3.6 mm) was smaller than the diameter of non-moyamoya vessels (3.8 mm), significantly (p= 0.042). However, there was no difference in the collateral patterns and clinical manifestation in a comparison of both groups. CONCLUSIONS: In our study, there was no significant difference of clinical manifestations and collateral patterns depend on the bony carotid canal diameter in patients with moyamoya disease. These findings suggest that the clinical presentations of moyamoya disease are not related to the onset time of the disease.
